Patient Benefits
Patient Benefits
Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) offer many benefits to the patients who choose to add these care options.
Healthcare Savings
Research shows that even though there is a small monthly coinsurance requirement, those enrolled in CCM spend less on their annual healthcare.
CCM offers cost savings that include:
Helping the patient find less-expensive prescription options.
Offering preventive care that prevents hospital visits and pricey care.
Immediate access to a nurse helps prevent emergency room visits for non-emergency issues.
Ability to ask qualified health professionals questions 24/7 without needing to schedule an office visit.
All of these benefits mean that someone enrolled in CCM can lessen their healthcare costs while improving their quality of care.
Improved Self-Management
The key to success for those with chronic conditions is proper self-management, which can be challenging for many to implement. What’s even harder is that if a chronic condition is ill-managed, it can become even more challenging to find the motivation to properly manage it due to pain, low energy, etc., further contributing to the cycle.
With CCM, patients are provided the resources and skills needed to properly care for their condition, giving them the confidence to make their health a top priority and the assurance that what they are doing is making a difference in their health.
Comprehensive Care
One of the biggest challenges those with chronic conditions face is coordinating care across all their doctors. With CCM, patients can feel confident that their doctors are working together to provide care.
Comprehensive care is more easily achieved with RPM too. Those enrolled in RPM provide doctors with a complete picture of their health, allowing them to see how certain physiological factors vary throughout the day instead of using a single snapshot taken in the office.
Some technologies used for RPM also have the capability to alert the doctor or care management team when a physiologic factor is outside the normal range, allowing them to recommend care for their patient as soon as they need it. This allows the doctor to play a more central role in the patient’s care while also ensuring that their health remains within the desired range.
Achieving Healthcare Goals
With CCM, patients discuss with their care management team their healthcare goals, which factor into the care plan created for them. With a set guide on what needs to be done, the patient can achieve their healthcare goals more easily. A CCM also keeps all members of the care team (i.e., patient, physicians, care manager) accountable while also providing a location for all members to reference and update care as needed.
