Serfiniti Wellness was established by Kemba R., a nurse with firsthand experience regarding the care and management needed for those with chronic conditions. She understands better than anyone the importance of proper care management and dedicates her time to ensuring that no one’s care becomes neglected.
The Need for Care Management
It is estimated by the Centers for Disease Control and Prevention (CDC) that 21% of Americans between the ages of 45 and 64 have two or more chronic conditions. Not only do chronic conditions require more extensive care than diseases that come and go, but their mismanagement is more likely to result in additional conditions that can negatively affect the individual’s quality of life. One of the biggest problems that those with chronic conditions face is coordinating care across all their medical needs since they often visit multiple doctors. A critical aspect of exceptional care is properly coordinating information between each medical professional, and that is where Serfiniti Wellness comes in.
Our Mission
Our mission at Serfiniti Wellness is to work with our patients to help manage all aspects of care needed for their chronic condition. With this goal in mind, we work to improve their quality of life while also reducing the likelihood of complications later. We care deeply for the well-being of all our patients and work hard to ensure that their health remains well-taken care of. Serfiniti Wellness works with physician offices and closely monitors the patient’s care outside the office. We take care of everything, including referrals, community outreach programs available to the patient, medication reconciliations, and other medical appointments.
